Bi-Est Cream is a compounded bioidentical hormone replacement therapy (BHRT) preparation that combines two naturally occurring estrogens — estradiol (E2) and estriol (E3) — in a smooth transdermal cream base. What is Bi-Est?

Bi-Est (sometimes written Biest) blends the two most clinically relevant human estrogens. Estradiol is the most potent estrogen, supporting bone, brain and reproductive tissues. Estriol, a gentler estrogen, is thought to provide protective effects for vaginal, urinary, skin and mucosal tissue. Compounded in an 80/20 estriol-to-estradiol ratio as standard (other ratios available on prescription), Bi-Est is designed to deliver symptomatic relief while tempering the stronger effects of estradiol alone.

Why transdermal?

Applied to the skin, Bi-Est is absorbed directly into the circulation, bypassing first-pass liver metabolism. This transdermal route may offer a gentler cardiovascular profile than oral estrogen and allows precise, individualised dosing in small concentration steps — from 0.25 mg/ml up to 10 mg/ml.

How to use

Apply the prescribed dose once or twice daily to clean, dry, thin-skin areas — typically the inner wrists, upper inner arms, inner thighs or lower abdomen — and rotate sites. Allow to absorb fully before dressing. Always follow your prescriber's directions and attend follow-up blood work.
