During testosterone therapy, three groups of tests anchor safe monitoring: early-morning total testosterone to confirm the dose is in range; haematocrit / haemoglobin to detect blood thickening; and cardiovascular risk markers including lipid profile and blood pressure.

Guidelines (Endocrine Society, AUA) call for an initial review within months of starting and then periodic reassessment. A haematocrit rising above threshold typically means a pause or dose adjustment. These intervals are not bureaucratic — they are the guardrails for safely continuing therapy.
